Dear Patient,

Renovo Chiropractic & Wellness is committed to protecting the privacy and security of our patients’ personal and medical information. We are writing to inform you of an incident that may have involved certain protected health information (PHI).

On January 16, 2026, our office learned that a break-in occurred at a storage facility where older administrative documents from our clinic had been archived. During this incident, approximately three years of Explanation of Benefits (EOB) documents were taken from the storage unit.

Explanation of Benefits documents are insurance records that may contain certain protected health information related to insurance billing. The information that may have been contained in these documents could include:

  • Patient name
    • Insurance identification number
    • Diagnosis codes associated with services billed

Importantly, the documents did not include Social Security numbers, phone numbers, email addresses, financial account information, or other private contact information.

Immediately after learning about the incident, Renovo Chiropractic & Wellness began an internal review to better understand the circumstances of the break-in and the type of information involved. We also reported the incident to law enforcement and notified our insurance carrier. In addition, we are reviewing and strengthening our document storage and record retention procedures to further improve how archived records are stored and protected.

At this time, we are not aware of any misuse of the information involved, but because the documents were removed during the storage facility break-in, unauthorized access to the information could have occurred.

 

What You Can Do

Because the information involved relates to insurance billing, we recommend that you review any Explanation of Benefits (EOB) statements or insurance statements you receive from your health insurance company. If you notice any services listed that you do not recognize or did not receive, please contact your insurance provider directly.

At this time, there is no indication that the information involved in this incident has been misused.

Protecting the privacy and security of patient information is extremely important to us. We regret that this incident occurred and are taking steps to enhance our storage and security procedures for archived records to help prevent similar incidents in the future.
